Download the FreedomTech Financial Loan Program Application and follow the steps below for completing the application (a printer will be required):

• The form may be completed either two ways:
  • The form contains fillable fields, so you may fill the form out from your PDF/Acrobat reader.
  • Print the application and then fill it out by hand (please print legibly).
• Complete all required fields and print out all pages (if you haven't done so already).
• Be sure that all applicants sign and date the Authorization page at the end with penned signature(s).
• Mail back the completed application in an envelope with postage to this mailing address: California Foundation for Independent Living Centers

• All done! A FreedomTech staff member will contact you to determine your status for a FreedomTech Financial Loan.
